Updating Your Voter Registration After a Name Change
If your name has changed, you must update your voter registration by completing a paper voter registration form. Be sure to include:
- Your new name
- Your address
- Your county
- Your birthdate
- Either your driver’s license number OR the last four digits of your Social Security number
- Your former name (in the appropriate section)
- Your signature and the date
Voting After the Registration Deadline
If the voter registration deadline has passed, your ability to vote depends on your situation:
- If you are already registered and have not moved out of your current precincg you may vote a regular ballot on Election Day by providing legal proof of your name change (e.g., a marriage license or court order showing both your current and prior names) You will need to fill out a registration form.
If you changed your name and moved out of your registered precinct OR do not provide proof of your name change (marriage certificate or court order), you will be required to vote a provisional ballot.
